CARE MANAGER POSITION Cape Atlantic Integrated Network for Kids is seeking care managers to provide care management services to youth in Cape May and Atlantic Counties. Responsibilities include facilitating Child Family Team meetings, developing and implementing Individual Service Plans, coordinating services and supports with families, behavioral health professionals, juvenile justice, DCPP and schools as well as conducting monthly face to face visits. Qualified individuals must have a Bachelors Degree in social work, psychology, or criminal justice with a minimum of one year experience working with youth and families or a Masters Degree in the above fields. Strong organizational, interpersonal and leadership skills are necessary. Computer skills required.
